Derby headquarters is at the Clearlake Oaks fire station, 12655 E Hwy 20. A map is available here. All fish will be weighed at Derby headquarders. NO FISH WEIGHED BETWEEN 11PM AND 8AM. LIVE FISH ONLY. |
| Entry Fee: | $50 per ADULT if received before 11PM Thursday of Derby week. |
| $60 per ADULT if received after that. | |
| $10 per KID 15 years and younger. | |

Entry forms will be accepted online, by mail or in person at the following locations: |
| - Limit Out Bait and Tackle, 12607 E Hwy 20, Clearlake Oaks | |
| - Derby headquarters (Thursday and Friday of Derby week) | |
| All entrants must pick up their contestant package and wristband in person at Derby headquarters before starting to fish. |
